Job Description

Hiring.zycto is actively seeking a dedicated and compassionate Case Manager – Social Work to join our growing team. This pivotal full-time role, situated within the vibrant community surrounding the Museum of Vancouver, offers an unparalleled opportunity to make a tangible difference in the lives of individuals and families in need. As a Case Manager, you will be at the forefront of providing comprehensive social work services, including assessment, planning, intervention, and advocacy for a diverse client caseload. This role requires a profound understanding of social determinants of health, mental health challenges, substance use, housing insecurity, and other systemic barriers faced by vulnerable populations. You will be responsible for developing individualized care plans, coordinating access to essential resources such as housing, healthcare, employment, and legal aid, and conducting regular follow-ups to ensure progress and adjust interventions as needed. A key aspect of this position involves collaborating closely with community partners, government agencies, and internal teams to ensure a holistic and integrated approach to client support.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify client needs, strengths, and goals.
  • Develop, implement, and monitor individualized care plans in collaboration with clients and their families.
  • Provide crisis intervention and de-escalation services as needed, ensuring client safety and well-being.
  • Connect clients with appropriate community resources, including housing, healthcare, employment, and mental health services.
  • Advocate on behalf of clients to ensure access to entitlements, services, and protection of their rights.
  • Maintain accurate and confidential client records, documenting all interactions and progress.
  • Collaborate with internal teams, external agencies, and community partners to ensure coordinated care.
  • Facilitate group sessions or workshops on life skills, self-advocacy, or wellness topics.
  • Participate in regular supervision, case conferences, and professional development opportunities.
  • Evaluate client outcomes and program effectiveness, contributing to continuous service improvement.

Required Skills

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current registration or eligibility for registration with the British Columbia College of Social Workers (BCCSW).
  • Minimum of 3 years of experience in case management or direct social work practice.
  • Demonstrated knowledge of crisis intervention, harm reduction, and trauma-informed approaches.
  • Strong understanding of systemic issues affecting marginalized populations.
  • Excellent written and verbal communication skills, with ability to articulate complex information clearly.
  • Proficiency in developing and implementing individualized care plans.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Exceptional organizational skills and attention to detail for case management and record-keeping.
  • Proficiency with client management software and standard office applications.

Preferred Qualifications

  • Experience working with diverse cultural groups and Indigenous communities.
  • Bilingualism or multilingualism is considered an asset.
  • Specialized training or certification in areas such as mental health first aid, addictions counseling, or conflict resolution.
  • Knowledge of local Vancouver community resources and social service networks.
  • Experience in program development or evaluation within a social work context.
