Key Responsibilities

  • Provide basic health education and information on preventive care, chronic disease management, and healthy lifestyles to individuals and groups.
  • Assist community members in accessing healthcare services, social support programs, and other community resources.
  • Conduct outreach activities to engage target populations and identify unmet health needs.
  • Serve as an advocate for clients, helping them communicate their needs to healthcare providers and navigate complex systems.
  • Document client interactions, progress, and outcomes accurately and confidentially.
  • Build and maintain trusting relationships with clients, community organizations, and healthcare partners.
  • Facilitate cultural mediation between clients and healthcare professionals to improve communication and understanding.
  • Participate in team meetings, training sessions, and professional development opportunities.
  • Identify and address social determinants of health affecting community members.
  • Distribute health-related materials and resources.

Required Skills
