Key Responsibilities
- Provide direct personal care to clients, including assistance with bathing, dressing, grooming, and personal hygiene.
- Assist clients with safe mobility, transfers, and positioning to ensure comfort and prevent injury.
- Support clients with feeding, hydration, and medication reminders as per individualized care plans.
- Observe and accurately report any changes in clients' physical condition, behavior, or mood to the nursing staff.
- Engage clients in meaningful social activities, conversations, and therapeutic exercises to promote mental and physical well-being.
- Maintain a clean, safe, and organized living environment for clients, assisting with light housekeeping tasks as needed.
- Accurately document care provided, observations, and client responses in accordance with facility policies and procedures.
- Collaborate effectively with an interdisciplinary team, including registered nurses, physicians, and other healthcare professionals.
- Adhere strictly to all health and safety regulations, infection control protocols, and confidentiality policies.
- Provide compassionate emotional support and companionship, fostering a sense of dignity, respect, and independence for clients.
Required Skills
- Completion of a recognized Continuing Care Assistant program or equivalent certification.
- Valid CPR and First Aid certification (Level C or HCP preferred).
- Demonstrated ability to provide compassionate, client-centered care with respect and empathy.
- Excellent verbal and written communication skills in English.
- Strong interpersonal skills with the ability to build rapport with clients and their families.
- Ability to work effectively both independently and as a valued member of a healthcare team.
- Keen observational skills and the ability to accurately document and report information.
- Physical capability to perform duties requiring lifting, transferring, and sustained periods of standing or walking.
Preferred Qualifications
- Minimum of 1 year of recent experience as a Continuing Care Assistant in a long-term care, home care, or assisted living setting.
- Proficiency in French (oral and written) is considered a significant asset due to our location in Quebec.
- Experience with electronic health record (EHR) systems for documentation.
- Training in Gentle Persuasion Approach (GPA) or Crisis Prevention Intervention (CPI).
