As a Home Care Nurse with Hiring.zycto, you will be empowered with autonomy and equipped with the resources needed to excel. Your daily responsibilities will involve traveling to multiple patient residences, performing comprehensive assessments, developing individualized care plans in collaboration with physicians and other healthcare professionals, and providing a wide range of skilled nursing interventions. This includes everything from advanced wound care, complex medication management, and intravenous therapy to palliative support, post-operative care, and chronic disease management. You will serve as a crucial advocate for your patients, educating them and their families on health management strategies, disease prevention, and ensuring seamless continuity of care between home and other healthcare settings. Key Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Administer medications, including injections and intravenous therapies, according to established protocols.
- Provide advanced wound care, ostomy care, and other specialized nursing interventions.
- Monitor patient vital signs, symptoms, and overall health status, reporting changes to physicians.
- Educate patients and their families on disease management, medication adherence, and health promotion.
- Manage and maintain accurate patient records and documentation using electronic health record systems.
- Collaborate effectively with interdisciplinary healthcare teams to ensure comprehensive patient care.
- Adhere strictly to all professional nursing standards, regulatory requirements, and company policies.
- Safely travel to multiple patient homes daily within the Flemingdon Park area and surrounding Toronto communities.
- Provide palliative care and end-of-life support as needed, demonstrating compassion and sensitivity.
Required Skills
- Current Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO).
- Valid Ontario driver's license and access to a reliable vehicle for travel.
- Minimum of 2 years of recent clinical experience as an RN, preferably in acute care or community settings.
- Demonstrated strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent verbal and written communication and interpersonal skills.
- Ability to work independently and manage a caseload effectively.
- Proficiency in using electronic health record (EHR) systems.
- Exceptional organizational and time management abilities.
- Commitment to patient-centered care and ethical practice.
Preferred Qualifications
- Previous experience in home care, palliative care, or community health nursing.
- Certification in specialized areas such as wound care, IV therapy, or gerontology.
- Experience working with diverse patient populations and cultural sensitivities.
- Bilingualism (e.g., English and Tagalog, Cantonese, Mandarin, or Punjabi) is an asset.
