Key Responsibilities
- Conduct comprehensive in-home health assessments and develop individualized care plans.
- Administer medications, perform wound care, and manage various medical treatments as prescribed.
- Monitor patient health status, identify changes, and report findings to physicians and the interdisciplinary team.
- Educate patients and families on disease management, medication adherence, and health promotion.
- Maintain accurate and detailed patient records and documentation.
- Collaborate with physicians, caregivers, and other healthcare professionals to ensure coordinated care.
- Provide compassionate emotional support to patients and their families.
- Adhere to all professional nursing standards, policies, and ethical guidelines.
- Manage a caseload effectively, prioritizing and organizing visits efficiently.
Required Skills
- Current Registered Nurse (RN) license in Ontario, Canada.
- Minimum 2 years of nursing experience, preferably in home care, community health, or acute care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in medication administration and wound care.
- Ability to work independently and manage a caseload effectively.
- Valid driver's license and access to a reliable vehicle.
- Current Basic Life Support (BLS) certification.
Preferred Qualifications
- Experience with electronic health records (EHR) systems.
- Certification in a specialty area (e.g., palliative care, gerontology).
