Key Responsibilities

  • Provide person-centred care, assisting clients with personal hygiene, dressing, and mobility.
  • Support clients with medication reminders and administration, following care plans strictly.
  • Assist with meal preparation, grocery shopping, and ensuring adequate nutrition and hydration.
  • Perform light household duties, maintaining a clean and comfortable living environment.
  • Offer companionship, engaging in conversation and activities to prevent social isolation.
  • Escort clients to appointments, social activities, or errands as required.
  • Accurately document care provided and report any changes in client condition or concerns to the care coordinator.
  • Adhere to all health and safety policies and procedures, including infection control.
  • Maintain professional boundaries and confidentiality at all times.
  • Work collaboratively with clients, their families, and other healthcare professionals to ensure integrated care.
