Key Responsibilities
- Administer medications safely and accurately, following established protocols and physician orders.
- Monitor, assess, and document vital signs, patient conditions, and responses to treatments.
- Perform various nursing procedures, including wound care, catheter care, and specimen collection.
- Assist patients with activities of daily living, promoting independence and dignity.
- Collaborate effectively with Registered Nurses, physicians, and other healthcare professionals.
- Maintain accurate and thorough patient records, ensuring compliance with documentation standards.
- Educate patients and their families on health management and disease prevention.
- Ensure a safe and therapeutic environment for all patients.
- Adhere to all organizational policies, procedures, and professional standards of practice.
Required Skills
- Current active registration as a Licensed Practical Nurse (LPN) with the College of Nurses of Ontario (CNO).
- Valid Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Minimum of 1 year of recent experience as an LPN in a healthcare setting.
- Excellent communication and interpersonal skills, both verbal and written.
- Demonstrated ability to assess, plan, implement, and evaluate patient care.
- Strong critical thinking and problem-solving abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health record (EHR) systems and basic computer skills.
Preferred Qualifications
- Experience in long-term care, acute care, or community health settings.
- Advanced certifications (e.g., IV therapy, wound care management).
- Bilingualism (English and other languages relevant to the Toronto community).
