Key Responsibilities
- Assist patients with activities of daily living (ADLs) including personal hygiene, dressing, grooming, and mobility.
- Provide companionship and emotional support, fostering a positive and comforting environment.
- Help with light household tasks, meal preparation, and feeding as needed.
- Monitor and report changes in patient condition, behavior, or needs to the supervising nurse or case manager.
- Assist with transfers and positioning, ensuring patient safety and comfort.
- Accompany patients to appointments or outings, as required.
- Maintain accurate and timely documentation of care provided.
- Adhere to all health and safety protocols, company policies, and patient privacy standards.
- Engage patients in stimulating activities and conversations and encourage independence where possible.
