As a Patient Navigator, you will be the compassionate guide for patients, helping them navigate the complexities of the healthcare system, ensuring they receive timely and appropriate care, and addressing their non-clinical needs. You will work closely with clinical teams, administrative staff, and most importantly, patients and their families, to foster a smooth and supportive journey through treatment and recovery. This role is designed for individuals with little to no prior experience in healthcare, offering comprehensive training, mentorship, and a clear pathway for professional development within our dynamic organization. Key Responsibilities
- Serve as the primary point of contact for patients, families, and caregivers, providing compassionate support and guidance throughout their healthcare journey.
- Assist patients in understanding appointment schedules, treatment plans, and necessary preparations, reducing anxiety and confusion.
- Facilitate communication between patients and various healthcare providers, including doctors, nurses, and specialists, ensuring clear information exchange.
- Help patients access necessary resources such as transportation, financial aid information, and community support services.
- Maintain accurate and confidential patient records, ensuring all interactions and support provided are meticulously documented.
- Provide administrative support to the healthcare team, including scheduling, data entry, and managing patient inquiries.
- Identify and address potential barriers to care, working proactively to resolve issues and improve patient access.
- Participate actively in ongoing training and professional development sessions to enhance knowledge and skills in patient navigation.
- Adhere strictly to all healthcare privacy regulations (e.g., PHIPA in Ontario) and organizational policies.
- Advocate for patient needs and preferences, ensuring their voices are heard and respected within the care process.
Required Skills
- High school diploma or equivalent.
- Exceptional interpersonal and communication skills, both verbal and written.
- Demonstrated empathy, patience, and a genuine desire to help others.
- Strong organizational abilities and attention to detail.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Basic computer proficiency, including familiarity with Microsoft Office Suite.
- A strong commitment to patient confidentiality and ethical conduct.
- Ability to learn and adapt quickly in a fast-paced healthcare environment.
Preferred Qualifications
- Post-secondary education in healthcare administration, social work, or a related field (e.g., Medical Office Administration certificate).
- Previous volunteer experience in a healthcare setting or customer service role.
- Bilingualism (English and other languages commonly spoken in Toronto, such as Mandarin, Cantonese, Punjabi, Tagalog) is a significant asset.
- Familiarity with medical terminology and healthcare systems.
