Key Responsibilities

  • Provide direct nursing care, including administering medications, performing treatments, and wound care, within the RPN scope of practice.
  • Conduct thorough patient assessments, document findings, and report changes in patient condition to the interdisciplinary team.
  • Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and other healthcare professionals.
  • Monitor and interpret vital signs, laboratory results, and other diagnostic data to inform care decisions.
  • Educate patients and their families on health maintenance, disease prevention, and medication management.
  • Maintain accurate and confidential patient records in accordance with professional and organizational standards.
  • Collaborate effectively with physicians, registered nurses, personal support workers, and other team members.
  • Respond to emergency situations and provide initial interventions as per established protocols.
  • Adhere strictly to all College of Nurses of Ontario (CNO) standards of practice, ethical guidelines, and organizational policies.
  • Participate in quality improvement initiatives and ongoing professional development activities.

Required Skills

  • Current registration in good standing as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO).
  • Valid Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
  • Demonstrated knowledge of nursing theory, clinical practice, and health assessment techniques.
  • Excellent critical thinking, problem-solving, and decision-making skills.
  • Strong communication (verbal and written) and interpersonal abilities.
  • Proficiency in administering medications and performing various nursing procedures.
  • Ability to work effectively both independently and as part of an interdisciplinary team.
  • Commitment to patient safety, privacy, and ethical practice.
  • Basic computer literacy and experience with electronic health record (EHR) systems.

Preferred Qualifications

  • Minimum of one (1) year of experience as an RPN in a community, acute care, or long-term care setting.
  • Additional certifications in specialized areas (e.g., IV initiation, wound care, palliative care).
  • Experience with specific patient populations (e.g., geriatrics, mental health, chronic disease management).
  • Fluency in a second language relevant to Toronto's diverse population (e.g., French, Mandarin, Punjabi).
  • Strong understanding of cultural sensitivity and diversity in patient care.
