Key Responsibilities
- Conduct comprehensive intake assessments to identify client needs, strengths, and goals.
- Develop and implement individualized service plans in collaboration with clients, ensuring their active participation.
- Provide crisis intervention, de-escalation, and emotional support to individuals and families experiencing distress.
- Connect clients with appropriate community resources, including housing, food security, employment, education, and healthcare services.
- Advocate on behalf of clients to access necessary services and navigate complex systems.
- Maintain accurate, confidential, and up-to-date client records and case notes.
- Prepare comprehensive reports and present case summaries to interdisciplinary teams.
- Collaborate effectively with internal staff, external agencies, and community partners.
- Facilitate group sessions and workshops on topics relevant to client well-being and life skills.
- Participate in ongoing professional development and team meetings to enhance service delivery.
Required Skills
- Social Service Worker Diploma or equivalent.
- Minimum of 2 years of direct experience in a social service or community support role.
- Proficiency in client assessment, case management, and crisis intervention techniques.
- Strong understanding of social determinants of health and community resources in Calgary.
- Exceptional interpersonal, communication, and active listening skills.
- Demonstrated ability to work effectively with diverse populations and cultural backgrounds.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and client management software.
- Ability to maintain strict client confidentiality and adhere to ethical guidelines.
- Valid Class 5 driver’s license and access to a reliable vehicle.
