Job Description

Key Responsibilities

  • Conduct comprehensive psychosocial assessments and develop individualized care plans.
  • Provide crisis intervention, brief counseling, and advocacy for clients facing complex challenges.
  • Facilitate referrals and navigate clients through community resources for housing, employment, healthcare, and education.
  • Maintain accurate, confidential, and timely client records in accordance with professional standards.
  • Collaborate effectively with interdisciplinary teams, external agencies, and community partners.
  • Participate in case conferences, program planning, and quality improvement initiatives.
  • Adhere strictly to the ethical guidelines and professional standards set by the OCSWSSW.
  • Engage in continuous professional development to enhance clinical skills and knowledge.
  • Empower clients to develop coping strategies, achieve personal goals, and enhance their well-being.

Required Skills

  • Master of Social Work (MSW) or equivalent from an accredited university.
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum of 3 years of post-MSW experience in a clinical, community, or healthcare setting.
  • Demonstrated expertise in psychosocial assessment, intervention, and case management.
  • Proven crisis intervention and de-escalation skills.
  • Exceptional verbal and written communication skills, with strong interpersonal abilities.
  • Proficiency in using electronic health records (EHR) and standard office software.
  • Demonstrated cultural competence and commitment to anti-oppressive practice.
  • Ability to work independently, manage a diverse caseload, and collaborate effectively within a team.
  • Valid G-class driver's license and access to a reliable vehicle for community travel.

Preferred Qualifications

  • Specialized training or certification in specific therapeutic modalities (e.g., CBT, DBT, trauma-informed care).
  • Experience working with diverse client populations, including newcomers, seniors, or youth.
  • Bilingualism (e.g., French, Spanish, Mandarin, Punjabi) is a significant asset.
  • Experience in program evaluation or quality improvement initiatives.
  • In-depth knowledge of local community resources and social services available in Toronto, particularly the Etobicoke area.
