Key Responsibilities

  • Provide person-centred care and support to individuals in their own homes or community settings.
  • Assist with daily living tasks, including personal care, meal preparation, medication prompting, and household management.
  • Support individuals in accessing community activities, appointments, and social engagements.
  • Promote independence and develop life skills, empowering individuals to achieve their personal goals.
  • Maintain accurate records and documentation of care provided and any observed changes.
  • Communicate effectively with individuals, their families, and other healthcare professionals.
  • Adhere strictly to all company policies, procedures, and relevant safeguarding guidelines.
  • Participate in ongoing training and development opportunities to enhance skills and knowledge.
