Key Responsibilities
- Provide person-centred support, assisting individuals with daily living activities such as personal care, meal preparation, medication management, and household tasks.
- Support individuals in accessing community resources, appointments, social activities, and employment or educational opportunities.
- Develop and maintain professional relationships with individuals, their families, and other healthcare professionals.
- Assist with mobility, transfers, and use of specialist equipment as required, ensuring safety and comfort.
- Document care provided, observe and report any changes in an individual's condition or circumstances to senior staff.
- Implement and adhere to individual care plans, risk assessments, and company policies and procedures.
- Promote independence, choice, and inclusion, encouraging individuals to participate in decisions affecting their lives.
- Engage in continuous professional development and attend mandatory training sessions.
Required Skills
- Minimum 1 year (12 months) of professional experience in a care or support role.
- Demonstrated empathy, patience, and a genuine desire to help others.
- Excellent verbal and written communication skills in English.
- Ability to work independently and as part of a multidisciplinary team.
- Strong understanding of safeguarding principles and person-centred care.
- Flexibility to work various shifts, including evenings, weekends, and holidays.
Preferred Qualifications
- NVQ Level 2 or 3 in Health and Social Care (or equivalent qualification).
- Experience working with individuals with diverse needs (e.g., learning disabilities, mental health conditions, physical disabilities).
- Full UK driving licence and access to a vehicle (desirable but not essential).
- First Aid certification.
