As a Social Services Case Manager, you will manage a caseload of clients, developing individualised care plans that address their unique needs, from housing and employment to mental health and family support. You will be responsible for conducting thorough assessments, coordinating services with various external agencies, and continuously monitoring client progress to ensure effective outcomes. We are looking for someone with an innate ability to build rapport, demonstrate empathy, and navigate sensitive situations with professionalism and discretion. Key Responsibilities

  • Conduct comprehensive intake assessments, including biopsychosocial evaluations, to determine client needs and eligibility for services.
  • Develop, implement, and monitor individualised care plans in collaboration with clients, setting measurable goals and objectives.
  • Coordinate and link clients with appropriate internal and external community resources, including housing, healthcare, employment, and legal services.
  • Advocate on behalf of clients to ensure access to essential services and resources, addressing systemic barriers.
  • Maintain accurate, up-to-date, and confidential client records in compliance with organisational policies and data protection regulations.
  • Regularly review client progress, adjust care plans as necessary, and facilitate client transitions or discharge planning.
  • Provide crisis intervention and support as needed, responding to urgent situations with calm and effective strategies.
  • Collaborate with multidisciplinary teams, participate in case conferences, and contribute to ongoing service improvement initiatives.
  • Stay informed about current social work practices, relevant legislation, and community resources to provide the most effective support.
  • Engage in continuous professional development and training opportunities to enhance skills and knowledge.

Required Skills

  • Proven experience in social work, case management, or a related field within social services.
  • Strong understanding of social welfare policies, practices, and resources in Scotland.
  • Excellent interpersonal and communication skills, both written and verbal, for diverse audiences.
  • Demonstrated ability to conduct assessments, develop care plans, and manage a caseload effectively.
  • Proficiency in crisis intervention, conflict resolution, and de-escalation techniques.
  • Ability to work independently and as part of a team in a fast-paced, challenging environment.
  • Strong organisational skills and attention to detail for accurate record-keeping and reporting.
  • Computer literacy, including experience with case management software and Microsoft Office Suite.
  • Valid UK driving license and access to a vehicle for client visits (desirable, but not strictly mandatory if public transport is viable).

Preferred Qualifications

  • A Bachelor's or Master's degree in Social Work, Psychology, Sociology, or a related human services field.
  • Registration with the Scottish Social Services Council (SSSC).
  • Experience working with specific vulnerable populations (e.g., homeless individuals, substance users, mental health service users, families at risk).
  • Knowledge of local Edinburgh community resources and service providers.
  • Training in trauma-informed care or motivational interviewing.
