Key Responsibilities
- Conduct comprehensive health assessments, including physical examinations and health histories.
- Diagnose and manage acute and chronic illnesses, including complex conditions.
- Order, interpret, and manage diagnostic and laboratory tests effectively.
- Develop, implement, and evaluate individualized evidence-based care plans in collaboration with patients.
- Prescribe medications and other therapies within the full scope of practice and regulatory guidelines.
- Provide comprehensive health education and counseling to patients and their families on disease prevention and management.
- Collaborate seamlessly with physicians, specialists, and other healthcare professionals to coordinate patient care.
- Participate actively in quality improvement initiatives, clinical research, and policy development as appropriate.
- Maintain accurate, detailed, and timely patient records in compliance with professional standards and organizational policies.
- Advocate effectively for patient needs and promote patient self-management and wellness strategies.
Required Skills
- Valid Nurse Practitioner (NP) license in Ontario (e.g., NP-PHC, NP-Adult, NP-Pediatrics, or NP-Critical Care).
- Master's degree in Nursing (MN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Minimum of 3 years of progressive clinical experience as a practicing Nurse Practitioner.
- Advanced physical assessment and diagnostic reasoning skills with a strong clinical acumen.
- Proficiency in prescribing and managing medications, including controlled substances, within scope.
- Exceptional communication, interpersonal, and multidisciplinary team collaboration abilities.
- Strong critical thinking, problem-solving, and decision-making skills in complex clinical situations.
- Demonstrated commitment to patient-centered care, cultural competency, and ethical practice.
- Ability to work autonomously, manage a diverse patient caseload, and prioritize effectively.
Preferred Qualifications
- Experience specifically in a primary care or family health team setting.
- Certification in a specialty area (e.g., gerontology, palliative care, mental health, diabetes education).
- Proficiency and familiarity with leading electronic health records (EHR) systems used in Ontario (e.g., TELUS PS Suite, Accuro, OSCAR).
- Bilingualism (English and other common languages spoken in Toronto, such as Mandarin, Cantonese, Tagalog, Punjabi, etc.).
- Experience precepting nursing students or mentoring new Nurse Practitioners/staff.
