Key Responsibilities

  • Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and feeding, ensuring their comfort and dignity at all times.
  • Monitor and accurately record vital signs (temperature, pulse, respiration, blood pressure), promptly reporting any significant changes or concerns to the nursing staff.
  • Help patients with safe mobility and transfers, utilizing appropriate equipment and techniques to prevent injury for both patient and caregiver.
  • Maintain a clean, safe, and organized patient environment, which includes tidying rooms, changing linens, and ensuring medical supplies are adequately stocked and accessible.
  • Provide emotional support and companionship to patients, fostering a positive, comforting, and reassuring atmosphere.
  • Assist with meal service, encouraging adequate nutritional intake and reporting any dietary concerns, difficulties with eating, or refusal of food.
  • Document care provided, observations, and patient responses accurately and timely in accordance with established facility policies and privacy regulations.
  • Communicate effectively and professionally with patients, their families, and the multidisciplinary healthcare team, relaying pertinent information and contributing to comprehensive care planning.
  • Respond promptly and efficiently to patient calls and requests, addressing needs or escalating to registered nurses as necessary.
  • Adhere strictly to all health and safety regulations, infection control policies, and patient privacy (PHIPA) guidelines to ensure a secure and confidential care setting.

Required Skills

  • Completion of a recognized Nursing Assistant or Personal Support Worker program.
  • Proven ability to assist patients with activities of daily living (ADLs).
  • Excellent communication and interpersonal skills, with a compassionate approach.
  • Strong observational and accurate reporting skills.
  • Ability to work effectively and collaboratively as part of a multidisciplinary team.
  • Demonstrated empathy, patience, and a genuine passion for patient care.
  • High level of reliability, punctuality, and a strong work ethic.
  • Basic computer proficiency for electronic health record documentation.

Preferred Qualifications

  • Previous experience (1+ year) in a long-term care, acute care, or community healthcare setting.
  • Current First Aid and Basic Life Support (BLS) certification.
  • Knowledge of medical terminology and common geriatric conditions.
  • Ability to speak multiple languages (e.g., French, Mandarin, Punjabi) is considered an asset in diverse Toronto communities.
