Key Responsibilities
- Assess individual and community health needs, developing and implementing evidence-based health promotion and disease prevention programs.
- Provide immunizations and manage immunization records in accordance with public health guidelines and protocols.
- Conduct comprehensive health education sessions for individuals and groups on topics such as nutrition, parenting, sexual health, and chronic disease management.
- Participate in communicable disease surveillance, investigation, and control activities, including contact tracing and outbreak management.
- Collaborate with community partners, healthcare providers, and social service agencies to coordinate care and resources for clients.
- Advocate for health equity and social justice, working to reduce health disparities within the Highland Creek community.
- Maintain accurate and confidential client records, ensuring compliance with privacy legislation and professional standards.
- Contribute to program planning, evaluation, and quality improvement initiatives within the public health framework.
Required Skills
- Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO).
- Bachelor of Science in Nursing (BScN) or equivalent from a recognized university.
- Demonstrated knowledge of public health principles, programs, and relevant legislation.
- Excellent assessment, planning, intervention, and evaluation skills for diverse populations.
- Strong communication, interpersonal, and teamwork abilities.
- Proficiency in critical thinking, problem-solving, and independent decision-making.
- Valid Ontario Driver's License and access to a reliable vehicle for community travel.
Preferred Qualifications
- Previous experience in a public health setting, particularly with community-based programs.
- Certification in Basic Cardiac Life Support (BCLS) or CPR Level C.
- Fluency in additional languages relevant to the diverse communities in Toronto.
