Key Responsibilities

  • Conduct comprehensive assessments of clients' physical, cognitive, social, and environmental needs.
  • Develop, implement, and regularly review individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Coordinate and monitor a wide range of services, including medical appointments, home care, transportation, and social engagement activities.
  • Act as a primary point of contact and advocate for clients, ensuring their preferences and rights are respected.
  • Provide ongoing support and education to clients and their families regarding geriatric conditions, available resources, and care options.
  • Assist with transitions of care, such as hospital discharge planning or moving to assisted living facilities.
  • Maintain accurate and confidential client records in compliance with professional and organizational standards.
  • Identify and address potential risks to client safety and well-being, implementing proactive solutions.
  • Facilitate family meetings and mediate discussions to ensure alignment on care goals and strategies.
  • Stay current with best practices in geriatric care management and relevant community resources.

Required Skills

  • Bachelor's degree in Social Work, Nursing, Gerontology, or a related healthcare field.
  • Minimum of 3 years of experience in geriatric care management, case management, or a similar role.
  • Proven ability to conduct comprehensive client assessments and develop tailored care plans.
  • Excellent interpersonal, communication, and negotiation skills.
  • Strong organizational skills with the ability to manage multiple cases efficiently.
  • Proficiency in Microsoft Office Suite and electronic health record (EHR) systems.
  • Valid Ontario driver's license and access to a reliable vehicle for client visits.
  • Demonstrated empathy, patience, and a client-centered approach to care.

Preferred Qualifications

  • Master's degree in Social Work, Nursing, or Gerontology.
  • Certification as a Geriatric Care Manager (GCM) or equivalent.
  • Fluency in additional languages relevant to the Toronto community (e.g., Mandarin, Cantonese, Punjabi, Tagalog).
  • Experience working with diverse populations and understanding of cultural sensitivities in elder care.
  • Knowledge of local Scarborough and Toronto community resources for seniors.
