Job Description

Hiring.zycto is seeking a passionate and driven Community Health Worker with a focus on Outreach to join our client’s dedicated team in Mount Olive, Toronto. This vital role is at the forefront of community engagement, acting as a crucial link between healthcare services and the residents who need them most. You will spend your days connecting directly with individuals and families in the Mount Olive neighborhood, building trust, and empowering them to navigate the healthcare system effectively. Your work will involve educating community members on available health programs, preventative care, and resources, ensuring access is equitable and culturally sensitive.

Key Responsibilities

  • Conduct proactive outreach activities in the Mount Olive community to identify individuals and families needing health and social services.
  • Build and maintain trusting relationships with community members, providing culturally sensitive and accessible health education.
  • Assist clients in navigating the healthcare system, including making appointments, understanding medical information, and accessing transportation.
  • Connect individuals to relevant community resources, social support services, and government programs.
  • Collect and maintain accurate client data and program statistics, ensuring confidentiality and adherence to privacy protocols.
  • Advocate on behalf of clients to address barriers to care and improve health outcomes.
  • Participate in community meetings, health fairs, and outreach events to promote health literacy and service awareness.

Required Skills

  • Proven experience in community outreach or health promotion.
  • Excellent interpersonal and communication skills (verbal and written).
  • Strong ability to build rapport and trust with diverse populations.
  • Demonstrated understanding of social determinants of health and health equity principles.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in basic computer applications (MS Office Suite, data entry).
  • Valid Ontario Driver's License and access to a reliable vehicle.

Preferred Qualifications

  • Post-secondary education in Community Health, Social Work, Nursing, or a related field.
  • Knowledge of local community resources and services in the Mount Olive/Etobicoke area.
  • Proficiency in additional languages relevant to the diverse population of Mount Olive (e.g., Punjabi, Urdu, Tagalog, Arabic).
