Key Responsibilities
- Provide person-centered care and support to clients in their homes and community, adhering to individual care plans.
- Assist clients with daily living activities, including personal care, meal preparation, medication management, and household tasks.
- Facilitate social engagement and community participation, supporting clients to access local services and activities.

Required Skills
