Hiring.zycto is seeking a dedicated Community Health Nurse specializing in Outreach to join our dynamic team in Signal Hill, Calgary. Key Responsibilities

  • Conduct comprehensive community health assessments to identify prevalent health needs, risk factors, and barriers to care within target populations.
  • Develop, implement, and evaluate evidence-based health promotion and disease prevention programs tailored to specific community groups and demographics.
  • Provide direct nursing care, health education, and counseling to individuals, families, and groups in various community settings, including homes, schools, and community centers.
  • Administer immunizations, perform health screenings, and conduct communicable disease investigations in accordance with public health guidelines and protocols.
  • Collaborate effectively with community partners, social services, healthcare providers, and other stakeholders to ensure comprehensive and coordinated client care.
  • Accurately document client interactions, health interventions, program activities, and outcomes using electronic health record systems.
  • Act as a client advocate, connecting individuals and families with appropriate health and social resources, and navigating complex healthcare systems.
  • Participate actively in team meetings, professional development opportunities, and quality improvement initiatives to enhance service delivery.
  • Travel within designated communities in Calgary to deliver outreach services and engage with diverse populations.
  • Monitor and report on trends in community health, contributing to strategic planning and program adjustments.

Required Skills

  • Active and unrestricted registration with the College of Registered Nurses of Alberta (CRNA).
  • Minimum of 3 years of recent experience as a Registered Nurse, with at least 1 year specifically in community health or public health settings.
  • Demonstrated knowledge of population health principles, health promotion strategies, disease prevention, and communicable disease control.
  • Exceptional assessment, critical thinking, and problem-solving abilities.
  • Superior interpersonal, communication (verbal and written), and public presentation skills.
  • Proven ability to work autonomously with minimal supervision, as well as collaboratively within a multidisciplinary team.
  • Valid Class 5 driver's license and consistent access to a reliable vehicle for extensive community travel.
  • Proficiency in electronic health record (EHR) systems and standard office software applications.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) with a specialization or significant coursework in Public Health or Community Health.
  • Certification in Community Health Nursing (e.g., Canadian Nurses Association (CNA) certification).
  • Experience working effectively with diverse cultural populations, vulnerable groups, and Indigenous communities.
  • Proficiency in an additional language relevant to the Calgary community demographics.
  • Previous experience in health program development, implementation, and evaluation.
  • Experience in crisis intervention and mental health support within a community context.
