Key Responsibilities

  • Perform comprehensive, evidence-based wound assessments, including thorough evaluation of wound etiology, precise measurements, and assessment of periwound skin condition.
  • Develop, implement, and rigorously evaluate individualized wound care treatment plans utilizing advanced dressings, therapies, and interventional strategies.
  • Administer specialized wound care treatments, including debridement, negative pressure wound therapy, and compression therapy, strictly in accordance with physician orders and established best practice guidelines.
  • Provide clear, concise, and empowering education to patients, families, and caregivers on effective wound prevention strategies, meticulous care techniques, and beneficial lifestyle modifications.
  • Collaborate seamlessly and effectively with physicians, registered nurses, physiotherapists, dietitians, and other allied healthcare professionals to ensure truly integrated and holistic patient care.
  • Maintain accurate, detailed, and timely documentation of all wound assessments, interventions performed, and observed patient responses within electronic health records (EHR).
  • Actively participate in quality improvement initiatives, regular audits, and relevant research efforts aimed at advancing wound care practices and patient outcomes.
  • Serve as a primary resource and mentor for other nursing staff, providing expert guidance on wound care best practices, protocols, and emerging techniques.
  • Adhere strictly to all infection prevention and control policies and procedures to minimize the risk of healthcare-associated infections.
  • Monitor patient progress diligently, identifying potential complications early and proactively adjusting care plans as necessary to optimize healing.

Required Skills

  • Current, unrestricted registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO).
  • Current certification in Wound Ostomy Continence Nursing (WOCN) or Canadian Association for Enterostomal Therapy (CAET), or an equivalent recognized advanced wound care certification.
  • Minimum of 4 years of recent, progressive clinical experience specifically in wound care.
  • Demonstrated expertise in complex wound assessment, accurate diagnosis, and the application of advanced treatment modalities.
  • Exceptional critical thinking, astute problem-solving, and sound clinical decision-making abilities.
  • Strong verbal and written communication, as well as superior interpersonal and patient education skills.
  • Proficiency in utilizing electronic health record (EHR) systems for documentation and information retrieval.
  • Up-to-date knowledge of current wound care guidelines, evidence-based best practices, and robust infection control principles.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) or a Master of Science in Nursing (MSN) degree.
  • Extensive experience working in a specialized wound care clinic or within an advanced practice nursing role.
  • Familiarity with local community resources and support networks for ongoing wound care.
  • Bilingualism (e.g., English and Mandarin or Cantonese) to effectively serve our diverse Agincourt community.
  • Demonstrated experience contributing to quality improvement projects or initiatives in wound care.
