Key Responsibilities
- Conduct comprehensive patient assessments to identify health needs, functional status, and psychosocial factors.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and the healthcare team.
- Coordinate and facilitate access to necessary healthcare services, community resources, and support systems.
- Advocate for patients' rights and needs, ensuring culturally sensitive and patient-centered care.
- Educate patients and families on disease management, treatment regimens, self-care, and preventative health measures.
- Facilitate smooth transitions of care between acute, sub-acute, long-term, and home care settings.
- Evaluate patient progress and modify care plans as needed to achieve desired health outcomes.
- Maintain accurate and timely documentation of all patient interactions and care activities.
- Collaborate with physicians, specialists, social workers, therapists, and other healthcare professionals.
- Identify and address barriers to care, developing strategies to overcome challenges in patient management.
Required Skills
- Registered Nurse (RN) licensure in Alberta, in good standing
- Minimum 3 years of clinical nursing experience, with experience in acute care, community health, or case management
- Strong understanding of the Alberta healthcare system and community resources
- Excellent communication, interpersonal, and negotiation skills
- Proven critical thinking and problem-solving abilities
- Demonstrated ability to work independently and as part of a multidisciplinary team
- Proficiency in electronic health record (EHR) systems and standard office software
- Valid Class 5 Alberta Driver's License and access to a reliable vehicle for travel
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Certification in Case Management (CCM) or equivalent
- Experience with chronic disease management programs or complex care coordination
- Proficiency in a second language relevant to Edmonton's diverse population
