Key Responsibilities

  • Provide direct patient care, including assisting with activities of daily living (ADLs), mobility, and personal hygiene.
  • Monitor and accurately record vital signs, intake, output, and other patient observations, reporting any changes to nursing staff.
  • Orient new patients and their families to the facility's services, policies, and care routines.
  • Assist patients in understanding and accessing local community resources relevant to their care and daily living (e.g., pharmacies, transportation, support groups).
  • Offer emotional support and active listening to patients and families during transitional periods.
  • Facilitate communication between patients, families, and the healthcare team to ensure clear understanding and continuity of care.
  • Maintain a clean, safe, and organized patient environment, adhering to infection control protocols.
  • Document patient interactions and observations accurately and promptly in electronic health records.
  • Collaborate with nurses, doctors, social workers, and other allied health professionals to ensure integrated relocation support.
  • Assist with scheduling follow-up appointments and transportation for new patients as needed.

Required Skills

  • Certified Nursing Assistant (CNA) certification or equivalent.
  • Minimum of 1.5 years (18 months) of experience in a clinical healthcare setting.
  • Exceptional interpersonal and communication skills, both verbal and written.
  • Demonstrated empathy, compassion, and patience.
  • Strong organizational skills and attention to detail.
  • Ability to work effectively in a fast-paced and diverse environment.
  • Proficiency in basic computer applications and electronic health records (EHR) systems.
  • Ability to work collaboratively as part of a multidisciplinary team.

Preferred Qualifications

  • Experience working with diverse patient populations and cultural backgrounds.
  • Bilingual proficiency, especially in Spanish or other languages common in NYC.
  • Knowledge of New York City's healthcare system and community resources.
  • Prior experience in patient advocacy, social work, or relocation support roles.
  • CPR and Basic Life Support (BLS) certification.
