Key Responsibilities
- Provide direct personal support and care to clients in their homes and within the community, adhering to individualized support plans.
- Assist clients with daily living activities, including meal preparation, hygiene, medication reminders, and household management.
- Facilitate and support clients' participation in community activities, social events, appointments, and recreational pursuits.
- Develop and maintain professional, supportive relationships with clients, their families, and other stakeholders.
- Monitor clients' well-being and progress, accurately documenting observations and reporting any concerns or changes to supervisors.
- Implement crisis prevention and intervention strategies as needed, ensuring client safety and well-being.
- Advocate for clients' rights and access to services, promoting self-advocacy where appropriate.
- Collaborate effectively with a multidisciplinary team to ensure holistic and coordinated client care.
- Complete and maintain accurate records, reports, and documentation in a timely manner.
Required Skills
- Post-secondary education in Social Service Work, Developmental Services, Psychology, or a related field.
- Minimum of 1 year of direct experience in community support, healthcare, or social services.
- Valid First Aid and CPR certification.
- Excellent interpersonal, communication (written and verbal), and active listening skills.
- Ability to work independently and as part of a team in a dynamic environment.
- Demonstrated empathy, patience, and a non-judgmental approach.
- Proficiency in basic computer applications and documentation software.
- Valid G-class driver's license and access to a reliable vehicle for community travel.
Preferred Qualifications
- Experience working with diverse populations and understanding of cultural competencies.
- Knowledge of local community resources and support networks in the Downsview area.
- Crisis Prevention Intervention (CPI) training or equivalent.
- Experience with case management and individualized support planning.
