Key Responsibilities

  • Conduct comprehensive virtual health assessments and develop individualized care plans.
  • Provide skilled nursing care, health education, and symptom management remotely.
  • Monitor patient progress, document observations accurately, and report changes in condition.
  • Collaborate with physicians, specialists, and other healthcare providers to optimize patient outcomes.
  • Educate patients and their families on disease processes, medication management, and self-care techniques.
  • Adhere strictly to all professional nursing standards, ethical guidelines, and company policies.
  • Utilize telehealth platforms and electronic health records (EHR) proficiently for documentation and communication.
  • Participate in case conferences and quality improvement initiatives.
  • Maintain strict patient confidentiality and privacy according to relevant regulations (e.g., PHIPA).

Required Skills

  • Current Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO).
  • Minimum 2 years of recent clinical nursing experience.
  • Exceptional clinical assessment and critical thinking skills.
  • Strong verbal and written communication abilities, particularly in a virtual environment.
  • Proficiency with computers, telehealth platforms, and electronic health records (EHR).
  • Ability to work independently, manage a caseload effectively, and prioritize tasks.
  • Empathy, patience, and a patient-centered approach to care.
  • Excellent problem-solving and organizational skills.

Preferred Qualifications

  • Bachelor of Science in Nursing (BScN) degree.
  • Previous experience in community nursing, home care, or telehealth.
  • Certification in a specialty area (e.g., wound care, palliative care, chronic disease management).
  • Experience with remote patient monitoring technologies.
  • Fluency in additional languages relevant to the Toronto community.
