Key Responsibilities
- Conduct comprehensive virtual health assessments and develop individualized care plans.
- Provide skilled nursing care, health education, and symptom management remotely.
- Monitor patient progress, document observations accurately, and report changes in condition.
- Collaborate with physicians, specialists, and other healthcare providers to optimize patient outcomes.
- Educate patients and their families on disease processes, medication management, and self-care techniques.
- Adhere strictly to all professional nursing standards, ethical guidelines, and company policies.
- Utilize telehealth platforms and electronic health records (EHR) proficiently for documentation and communication.
- Participate in case conferences and quality improvement initiatives.
- Maintain strict patient confidentiality and privacy according to relevant regulations (e.g., PHIPA).
Required Skills
- Current Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO).
- Minimum 2 years of recent clinical nursing experience.
- Exceptional clinical assessment and critical thinking skills.
- Strong verbal and written communication abilities, particularly in a virtual environment.
- Proficiency with computers, telehealth platforms, and electronic health records (EHR).
- Ability to work independently, manage a caseload effectively, and prioritize tasks.

- Excellent problem-solving and organizational skills.
Preferred Qualifications
- Bachelor of Science in Nursing (BScN) degree.
