Key Responsibilities
- Assist Case Managers with administrative tasks, scheduling, and documentation.
- Organize and maintain patient records, ensuring accuracy and confidentiality.
- Facilitate communication between patients, families, and healthcare providers.
- Help coordinate appointments, referrals, and transportation for patients.
- Support in identifying and accessing community resources for patients.
- Prepare and process necessary paperwork and forms.
- Answer phone calls and respond to inquiries with professionalism and empathy.
- Adhere to all privacy regulations (e.g., PHIPA in Ontario) and company policies.
- Participate in team meetings and contribute to care planning discussions.
